Remember, a care home (rest home) is your home; just like any home you live in, you can choose to move if you wish.
The first step is to contact your local Needs Assessment Coordination Service (NASC) to let them know you want to transfer. They may do another interRAI assessment to make sure you’re moving to a care home that provides the appropriate level of care. Your NASC team will also complete any required documentation.
Your Admission Agreement (the legal document you signed when you moved into your current care home) will outline the notice period you will need to give the rest home you are leaving. Remember – you have to pay for the cost of your care, so try to arrange your transfer so that you don’t have to pay for two care homes.
Under their contract with Health New Zealand | Te Whatu Ora, a rest home, hospital or care facility must make it easy for you to transfer. If you have a problem, contact your local NASC for help.
