Sometimes families are living far apart, this may be geographically, it may be emotionally, or it may be due to loss.
Unfortunately this sometimes means an older person can be living at home with very little support. If living at home becomes too challenging, or they have an event that means living independently is no longer an option, family can't always be there either fast enough, or for long enough to be of full support right throughout the process. This is where Senior Transitions will act as your advocate, your eye's and your ears.
Service Description
Your families eyes and ears: We can attend medical appointments to advocate for outcomes, ensure clarity of communication and sharing of the right information. We always wish to meet with individuals prior to appointments to ensure comfort and to discuss preferred outcomes. Finding the next home: We can find, and inspect available rest homes and retirement facilities, as well as arrange visits for you (including transport if that helps). We can then assist with booking and admission arrangements. Documentation: We understand that all of the admission, financial and legal paperwork can be daunting, so we can walk you through it step by step. Moving: We will gladly make all of the arrangements to move you into your new home, ensuring that it is ready and waiting for you to arrive. Your property: We can arrange for your belongings to either move with you, be sold, go into storage for later, or be donated where possible. We can get your home ready for the next steps such as cleaning, gardening and staging, and we can then make arrangements for your real estate needs if your property is to be sold. |

Meeting the needs of older people
From a 20 year career in Transport, Logistics and Manufacturing Management, followed by joining his wife Rebecca to run New Zealand's most award winning Yoga and Pilates business, Jeremy has the ability to get things done, while maintaining integrity, empathy and compassion. Logistics, management and a client first mentality mean Jeremy is the ideal person to assist you or your loved one when they need support. Senior Transitions began after we were involved in transitioning a family member from independent living, to a Rest Home. During the process we noted the challenges faced when someone has to do this alone, and many conversations reinforced the idea that there needs to be some independent assistance available for those who don't have immediate support. |
