Rem, who is 88 was living alone when he was admitted to Dunedin Hospital in August. He was very unwell spending time in the Intensive Care Unit before being transferred to the medical ward.

It was in the medical ward that Occupational Therapist, Mii met Rem. His notes said ‘for residential care,’ but after spending some time with him and getting him up and moving she asked him what he wanted to do.

Rem told Mii that he wanted to stay in his own home, but he had almost resigned himself that he had to go into residential care.

Rem had already been referred to the Care Coordination Centre which asked the Needs Assessment Service Coordination team (NASC) to work with Mii to find a way to get Rem back home.

Mii says one of the barriers was that Rem is very hard of hearing and this meant that he often couldn’t convey his views or thoughts to the clinical team.

“I just needed to connect with him and soon realised that he really wanted to stay in his own home.”

Rem was able to go home with support. He was discharged to his former family home, but due to stairs and a large garden it was difficult for him to manage. Mii wrote a letter of recommendation to Housing New Zealand for Rem to be re-housed in a more suitable unit and he moved about a month after discharge from hospital.

Rem is now happily settled and living independently in a single storey one - bedroom unit with support from carers from Royal District Nursing service (RDNS) every day in the morning and evening.

He says he feels a lot more confident and safer and hasn’t had any falls since moving. He manages to cook himself and continues to with his social activities including attending church, on a Sunday, an exercise class at Te Kāika Caversham every Tuesday, and singing to the Ukele at Te Roopu Tautoko Ki Te Tonga every Thursday.
